<p>WEBSITE:wysil.org </p><p>COST: No Cost </p><p>SERVICES: Independent Living: Helps all individuals with a disability identify barriers to living independently and then develop a step by step plan to overcome the barrier. The goal of this program is to educate each consumer on all of the options that are available to them so that the consumer can make the best choice for their own independence </p><p>Project OUT: A short-term program designed to assist individuals transition out of or divert from a nursing home to live independently in their community. Medicaid eligibility is a requirement for this program. </p><p>Consumer Directed Care: Helps eligible consumers to recruit, hire, and supervise self-help assistants to assist with relevant aspects of day-to-day living. Individuals must be eligible for Medicaid Long Term Care Home and Community-Based Services Waiver. </p><p>Rural Transportation Voucher Program: Eligible consumers can receive rides from volunteer drivers and participating public transit operators. WSIL, through a grant from WYDOT, then reimburses the provider of the ride at a predetermined rate. </p><p>Independent Living Loan Closet: WSIL maintains a Loan Closet for use by anyone with a disability who needs specialized equipment. The Loan Closet has items such as canes, crutches, walkers, hospital beds, bedside commodes and wheelchairs which are loaned on</p><p>LOCATION: Western Half of Wyoming </p><p>HOURS: Monday-Friday, 8am -5PM </p>
